The 'Request for interview adjustments' forms part of the Disabilty and Mental Health portal. For student who has been offered a place at Kingston University, yet may require adjustments for their interview. It was important to keep the questionaire 'achievable' at first glance; yet with calls for action if further details is required from the student.
Nice touch
The list of quesions are 'easy on the eye' when going through the questionaire and only when an answer, which require further information, is a section with further details revealed.

Admin area is protected by University Access Manager and access restricted by list of KU ID in the database that requires access to this section of the backend.
